If you prefer to pay by cash or check, please present this to Caleb before Camp. Checks can be made out to Lifepointe Church, and please put "Camp" in the Memo. 

*WAIVER: Everyone that is attending will need a Camp Dakota Visitor Agreement signed (minors & adults) to participate in camp activities. Once you complete the form you will receive an email confirmation. Please forward the confirmation to caleb@lifepointecommunity.com ASAP.
