**NOTE - THE NAME AT TOP OF PAYMENT FORM IS THE PARENT NAME**

ALL MDO PAYMENTS MUST BE MADE ONLINE, DUE ON THE 1st OF EACH MONTH. (Late fee is $10). 

Monthly tuition is $60 per child / per day, plus the $2 processing fee.

If a child is enrolled for two days, the tuition is doubled.

Annual registration is $50.
