The HCPH Diabetes Prevention Program is a community-focused health initiative aimed at helping individuals reduce their risk of developing type 2 diabetes through lifestyle changes. Held on Wednesdays at 11:30 AM at Fallbrook Church, the program offers support, education, and practical tools to promote healthy eating, physical activity, and long-term wellness—led by Harris County Public Health professionals.
