Complete Your DonorExpress Health History Questionnaire online the day of the blood drive! It takes only about 10 minutes to answer the yes/no health history questions. DonorExpress will save you time at the blood drive.

It MUST be completed the day of your donation!

Go to vitalant.org/donorexpress

Click on "DONOR EXPRESS" under the "DONATE" tab at the top of the page.

Read the questionnaire instructions and check that you read the materials.

Select English or Spanish.

Go through and answer initial questions. Birth-date is in the form of mm/dd/yy (just last two digits of year).

Answer all medical questions then review your answers at the end.

Click on "Generate Donor Pass" either print out the results or email them to your mobile device.
